Client-centered coaching

Following an initial assessment, I employ a strengths-based approach towards developing a course of action. This takes into consideration the client’s personal strengths and seeks to resolve problems by nurturing the positive aspects of a client’s life. This can also include identifying positive interpersonal relationships, as well as outlining and supporting realistic and achievable goals, and charting out a path forward for the client, complete with reasonable and actionable steps. Clients are supported on a regular, ongoing basis.

 

Client-centered coaching can help:

  • Create and manage care teams (psychiatric, medical, exercise, etc.) based on the client’s unique needs

  • Act as a liaison between providers and or families in order to ensure comprehensive and collaborative care

  • Create feelings of success by scaffolding goals into small, achievable steps

  • Support transitions towards independence through the development of healthy behaviors and daily routines

  • Reengage with stable and diverse sources of positive reinforcement from their environment

  • Manage crisis situations

  • Establish appropriate client-specific plans for transitions to the next point of care
