Client-centered coaching
Following an initial assessment, I employ a strengths-based approach towards developing a course of action. This takes into consideration the client’s personal strengths and seeks to resolve problems by nurturing the positive aspects of a client’s life. This can also include identifying positive interpersonal relationships, as well as outlining and supporting realistic and achievable goals, and charting out a path forward for the client, complete with reasonable and actionable steps. Clients are supported on a regular, ongoing basis.
Client-centered coaching can help:
Create and manage care teams (psychiatric, medical, exercise, etc.) based on the client’s unique needs
Act as a liaison between providers and or families in order to ensure comprehensive and collaborative care
Create feelings of success by scaffolding goals into small, achievable steps
Support transitions towards independence through the development of healthy behaviors and daily routines
Reengage with stable and diverse sources of positive reinforcement from their environment
Manage crisis situations
Establish appropriate client-specific plans for transitions to the next point of care
